St. Joseph's/Candler’s Center for Medication Management has four convenient locations, as well as clinical pharmacists in primary care offices around the region, to help you with your medication needs and concerns. Our primary objective is to assist you in the better understanding of your medications through disease state education and knowledge, proper use and administration of the medication, monitoring parameters and specific therapy related goals. Our ultimate goal is to improve the standard of care with regard to medications, to reduce adverse side effects and drug interactions, improve the coordination of care among healthcare professionals and reduce hospital readmissions.

- Anticoagulation (Warfarin/Coumadin, new oral anticoagulant) management
- Diabetes management in conjunction with the Center for Diabetes Management
- Hyperlipidemia/Hypertension wellness
- Flu vaccinations
- Pneumonia vaccinations
- Care coordination among your physicians
- Tobacco Cessation Program
In August 2016, the St. Joseph’s/Candler Center for Medication Management earned the designation of Anticoagulation Center of Excellence from the Anticoagulation Forum for providing the highest level of care to patients taking antithrombotic medications. Read more about that here.
