Looking for lab results or doctor's orders? Want to pre-register for an upcoming appointment? St. Joseph's/Candler has an easy-to-use portal to securely and easily navigate your healthcare information. 

What do you get with the St. Joseph's/Candler patient portal? Just some of the services include easy access to your hospital health record, medication list, appointment reminders and send messages to your provider.

There are two ways to access the St. Joseph's/Candler Patient Portal: from your web browser and from our brand new mobile app.

Instructions for patient portal app

Here's a step-by-step guide to take full advantage of the app:

  1. How to find and download the MEDITECH MHealth Patient Portal app
    The MEDITECH MHealth Patient Portal app is the mobile version of the SJ/C patient portal formerly called iConnect. It is available for both Apple and Android users in the Apple or Google Play app stores. For Android users, you may also go to play.google.com and search for MHealth. 
  2. Download the MEDITECH MHealth Patient Portal app
    Once you go to the appropriate app store for your device, search for MHealth and select to download or install. The app will look like this:
  3. After downloading the MEDITECH MHealth app, you will need to locate the St. Joseph's/Candler patient portal
    In the top left, click on the Choose Portal and then tap "Near Me." The St. Joseph's/Candler patient portal will come up in a list of portals. Click on the one with the SJ/C logo. You can also find the St. Joseph's/Candler patient portal by selecting US states, then selecting Georgia and scrolling to St. Joseph's/Candler.
  4. Sign into the SJ/C patient portal
    After opening the app, you will be taken to the patient portal sign in page. Login with the same user name and password you have used for iConnect (new MEDITECH MHealth) in the past. If you do not have a user name and password, click create account.
  5. Explore and interact
    All information previously in your iConnect account will be available in the MEDITECH MHealth patient portal app with a refreshed look and feel.


If you are a family member or caregiver and need access to patient health information:

  1. Send an email to portalhelp@sjchs.org to request the setup of a proxy account
  2. HIM (Health Information Management) will send you a form via email that you will need to fill out and return
  3. Once the form is completed you will be able to set up the proxy account in the Patient Portal
