Preparing for your Outpatient Rehabilitation Visit

Thank you for choosing St. Joseph's/Candler Outpatient Physical/Occupational Therapy.  For your convenience, you may download and complete the following forms and bring with you to your first appointment.

We would also like to extend an invitation to your family and friends to accompany you in your therapy appointments.  Your therapy team welcomes guests to be involved in your program.  If you would like to invite a family or friend, please notify your therapist. 

If the patient is under 18, a legal guardian must accompany them to the registration appointment.
